How do I sign my consent form and health questionnaire?
A link sent after your request or deposit opens the form: identity details, health questionnaire, risk information, and signature.
A link is sent to you in the app or by email, or displayed as a QR code by the artist at the studio. It opens the consent form, which you can complete in a few minutes without having to log in again.
The form opens regardless of how you booked your appointment, whether through assignment by Tattooed or direct booking on an artist's page. The session triggers it, not the way the session was booked.
When the form is offered
If your deposit has already been paid, the form is offered immediately afterward on the confirmation screen and by email, with the option to complete it later if you prefer. If you booked directly with an artist through their public page, the form is offered as soon as you send your request, even before the artist accepts it. If the artist ultimately declines your request, your answers are deleted that same day. They are never kept for an appointment that did not take place.
What the form asks for, in order
First, it asks for your identity, which is partly prefilled, including your date of birth. Next comes a multiple-choice health questionnaire with a free-text field for current treatments. Then you see the legal information about tattoo risks, followed by six checkboxes, none of which are checked in advance: the permanent nature of a tattoo and the limits of laser tattoo removal, which is long, painful, and costly; the risk of infection despite sterile, single-use equipment; the risk of an ink allergy, which can arise years after the session; the uncertainty of healing and possible differences from the original design; confirmation that you received the aftercare instructions and your commitment to follow them; and verification of the design, its size, placement, and spelling, which alone prevents half of the disputes in the profession. Finally, you draw your signature with your finger, and it is timestamped.
You are also offered an optional checkbox to allow photos of your tattoo to be published once it is finished. This is separate from the permission related to photos you upload to the app for your project.
If you are a minor
Your date of birth is compared with the session date. If you are a minor on that date, the form stops after the identity step. The law requires your legal representative to sign in the artist's presence, which a checkbox cannot replace. A parental authorization form is then generated for printing, and the artist collects and uploads it from their pro dashboard.
Once it is signed
The form becomes final and can no longer be changed. If a correction is needed, a new form is opened and linked to the previous one, without ever rewriting the document that was already signed. Your answers remain visible to the artist concerned and to Tattooed as part of the legally required retention of the record.
